Program Guidelines
SOUTHWEST NEBRASKA DENTAL CENTER MEMBERSHIP AGREEMENT
This dental savings plan provided by Southwest Nebraska Dental Center is designed to provide affordability and greater access to quality dental care. Providing a dental savings plan is a viable alternative to conventional dental insurance.
Our plan is simple.
The fees paid for each plan include standard of care services and savings when paying in full at the time of service.
Annual fee of $199 for Child (13 and under) (Your savings: $120 plus a year)
Annual fee of $299 for Basic Adult (Your savings:$99 plus a year)
Annual fee of $675 for Perio 4 visits in a year (Your savings: $275 plus a year)
Participation begins on the date you pay your dental savings plan premium. Membership is ONE year from the signup date. You are given two options for payment: pay in full at the day of signup or monthly auto payments are available. *Monthly auto-payments are full ONE-year commitments and cannot be cancelled prior to the annual anniversary date.
Financial obligations must be complete prior to receiving the benefits of your savings plan. If the plan participation does not utilize the dental savings plan benefits or enrolls in a conventional dental insurance subsequent to their enrollment in the dental savings plan, no refunds will be issued or carried over to the next renewal year.
The participant will be notified of the renewal date approximately one month prior to the one-year anniversary date. After-which, the plan will automatically renew. To cancel or make changes to the dental savings plan you must call prior to the anniversary date.
The participants portion of any bill is due and payable on the date of service to receive the 15% dental savings. Interest free payment options for treatment needed/provided are available upon request and approved credit. Interest free options are 6,12,18-month durations and extended payments also with interest and credit approval are available through a 3rd party. Repayment duration is based on the totals and procedural type. If you choose to use a repayment/financial plan, your Dental Savings will be adjusted around the third-party administrative fee. Interest fee payment options or extended will be customized for your repayment needs. This means that the 15% savings is lowered to approximately 10% to cover the 3rd party administrative fees.
Please notify our office at least 48 hours in advance if you must change a reserved appointment.
Benefits are only available at Southwest Nebraska Dental Center 411 West 5th Street McCook Nebraska. This program is a dental savings plan and CANNOT be used in conjunction with conventional dental insurance. Please refer to the details on the swnebraskadental.com website.
